Arkansas law requires all students attending Arkansas schools to be vaccinated against certain diseases unless an official exemption form is filed.
Students eligible for immunization exemptions must be renewed each school year and will need to be applied for during the summer prior to the new
school year. The Arkansas State Health Department will provide assistance in applying for exemptions and can be contacted at 870-246-4078 or on-line at https://www.healthy.arkansas.gov/.
As of September 15, 2022, your child may not attend school until this deficiency is corrected or an exemption is provided. Please bring or send appropriate documentation to the school nurse.
If your child is unable to receive the required immunizations by September 15, 2022, then a scheduled appointment date and time will be needed for the child to remain at school. Call the school nurse with that date and time for the appointment.
